Initial Consultation |
Your initial consultation will take about 45 minutes. To save yourself some time, we have provided an online registration form for you. Just click on patient registration, fill out the form and click submit. Our office staff will have your completed registration form in your file before your first visit. This saves you office time and allows you to fill out the information in the comfort of your own home with all the information you need at your fingertips. If you cannot fill out the form ahead of time, please arrive a few minutes early to fill out the form in the office. If you don't have recent X-rays, plan an additional 20 minutes for your visit.
In a non-emergency, Dr. Pannullo will begin by examining your mouth and having a dental assistant record the information in your chart. He will tell you what is happening in your mouth and then will discuss with you the treatment options he believes are necessary. No treatment will begin at this time, but an appointment will be arranged for you.
In an emergency, Dr. Pannullo will treat you before you do anything else.
Please assist us at the time of your initial visit to the office by providing the following information:
- Your referral slip
- Recent X-rays from your referring dentist (or have your dentist forward them to us prior to your appointment)
- A list of medications you are currently taking
- Any forms or insurance cards
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
